I am concerned that information contained in charting is not correct. I understand that there is an outline for areas (SOAP) but the continual inconsistencies in reports are concerning to me. On visit 4/19/2024 MRI Femur - there were no markers place, yet it is indicated in the report there was. On visit 4/19/2024 abdomen was not checked, yet it states it was. Previous visits, multiple times more of the same - surgical report (that was finally changed to indicate I had a pneumothorax, previously stated all unremarkable) and other incidents the same with Internal Medicine department. This does not make patients feel comfortable. Either do the work that is reported OR don't do it, but don't template the records for everyone. You should know that charting is a big deal. I come to Emory because I have faith and trust in Dr. Cardona and Dr. RIzzo, but to me, the charting issues concern me
