I'm going to be transferring the maintenance of one of my meds to Dr. Tong bc she is so much easier to contact and communicate with than my endocrinologist. In addition, she listened to my concerns regarding some other medicine and we were able to come up with a great plan for management.

Since I have AD/HD, it is most helpful to me to have a written report of discussion & recommendations. I cannot emphasize this enough b/c a poor working memory is almost always part of an AD/HD diagnosis. In the moment, recall does not seem to be a problem, but later it is the reality. Yes, it does require more planning and follow through for the doctor, but the benefit to the patient to have a reliable record of medical issues will provide critical information for understanding and follow up which could otherwise evaporate.
